How big is the crate? If the puppy has lots of room to move around, they are more likely to go to the bathroom in it because they wouldn't be "technically" soiling their sleeping area. If the crate only has room for sleeping, they will be less likely to soil it because dogs want a nice place to sleep.

As people have already stated the crate may be to big.
Also the Crate doesn't act as a magical cork eliminating any need to go... it doesn't stop the dog from going, it just stalls if for a while. If they are desperate enough they will go where ever whenever. You can get a "floor" that goes into the crate that lifts the dog off the bottom, so that if he does go he is not rolling in it all day long while you are gone.
Getting on a more regular schedule helps. You may want to wait for a day off of work and take him out every 30 min to an hour. Therefore when you do "catch him" going outside you can reward him like crazy!

You are going to have to take him out of the crate and out for walks more frequently. When you do get him to go outside, give him LOTS of praise and play.
If you have to work, you may want to get a dog walker.
Especially puppies should not be in crates for long stretches without coming out. They cannot hold it and also they need a lot of exercise.
Dogs will not willingly pee/defecate in their own den/crate, so if he is going in there it is because he can't help it. He probably holds it as long as he can.
Crating is generally a good training tool, but it is commonly overused and dogs end up spending way too much time in a crate. Never should be in a crate more than eight hours total, and during those eight hours should have at least three walks.

Get a smaller crate or put in a partition to reduce his room in the crate. Sounds cruel, but a puppy will hold out a lot longer if he has to sit in his mess. Your crate is too big, the puppy can do his business and still have room to lay/sit down without getting messy.

The crate may be too big for him if he can get away from the mess. If he has to step or lay in his mess it will discourage him.
He is probably spending too much time in the crate--if you are home he should not be crated. As soon as you come home he should be IMMEDIATELY be taken out to relieve himself. Think about it--when you have too pee badly and know the bathrrom is just steps away your bladder starts to relax even before you get there. Hearing you he may begin to anticipate a trip out and if you delay he can't wait.

give him a smaller crate, and 3 times a day for a puppy isn't enough! (i'm sure you don't just take him out 3 times, but it sounds like it in your ??) The crate a puppy is in should only be big enough for him to lay down in and turn around in. There shouldn'nt be extra room in there. Is he going when you take him out? Is there food in the crate/water? Try (if not already) feeding him in the morning and at night, only! You will be able to 'judge' when he really needs to go!

His crate may be too big and he will relieve him self in one end and stay in the other. Try blocking off one end to shorten the crate. He will hold off going to the bathroom in his sleeping area. Also take him outside as much as you can till he understands that outside is the appropriate spot to go.

You should probably just give him more time to develop good bladder control. Puppies are little and often can't hold it in for very long. Is there any way you can take him outside more often? If his bladder is empty, he won't be able to relieve himself inside the crate.

My vet told me that if the crate is too big for the puppy they will go to the bathroom in it. She told me to get one just big enough for the puppy. Just so he could stand up and move around. Or maybe you just have him in his crate to long.
