Nobody likes oil spills.

There are two main approaches to avoiding oil spills, and other industrial accidents for that matter:

1) Hire the most competent operators and properly train them in their responsibilities.

2) Design and implement robust systems that can survive the most challenging situations, including human error and extreme weather conditions.
