There have been 5 5-star generals in the Army. They are: George C. Marshall, Douglas MacArthur, Dwight D. Eisenhower, Henry Arnold (although he later transferred to the Air Force on its inception) and Omar Bradley. Pershing and Washington were given the rank (title) of General of the Armies but they are considered to be 4-star officers. (5-star officers are in the rank of General of the Army.)

There have been 11 5-star flag officers in America's history
Four generals, four admirals. One air force general, Hap Arnold was awarded a five star designation twice, once in 1944 when it was still the Army Air Force, and again in 1947 when the AAF was redesignated into the USAF

The fifth star is a special award and doesn't happen very often. I believe WW II awards were the last ones, and you pretty much have to be in charge of an entire theater to get it, and only one person in that theater can have it.

These are the way "general" ranks go:

Brigadier general
Major general
Lieutenant general
General
General of the Army
