"After the anaesthesia takes effect, the doctor makes a small incision on the skin above the pubic bone. This is usually a horizontal incision, but can also be vertical. Layer by layer, the tissue is incised until the uterus is reached. The lower section of the uterus is cut horizontally. Only rarely, usually when the baby is premature, is a vertical cut made."

Although I haven't had a c-section, I have had major abdominal surgery with a vertical cut. I was told that this is more difficult to recover from, since it has a much greater impact on the horizontal girdle of muscles which support your back. I have had considerable back pain since the surgery. Of course, the scar would also be more visible if you want to wear bikinis.

1)Accessibility 2)extendability 3)Safety and 4 )Beauty are the crieteria of a good surgical incision.
Through a vertical incision that can be extended up or down if necessary, muscle fibres and many blood vessels which run vertically are saved from being cut accross. Post oprative hernia and excessive bleeding is thus prevented. Appendicectomy is routinely done during a Caesarian and Appendix is more accessible through a vertical cut.A vertical cut is more cosmetic . Horizontal incision for Caesarian is extremely rare and given for EMERGENCY CAESAREAN.

The ones you described are the classical and pfannenstiel methods of skin incision in cesarian section. The basic difference of the two kinds is only in the skin incision. The uterus underneath is still cut in a low transverse section. The pfannenstiel is preferred by mothers who generally need or want to show their abs after giving birth, as the skin cut is done on the bikini line. The result of the classic vertical incision is very unsightly, and would limit the wardrobe of the mother in the future. However, pfannenstiel is only done on a scheduled delivery, since it requires more time to dissect the muscles underneath because of the direction of the skin incision, and as such, cannot be used in emergent deliveries, where the vertical incision is employed.

the classic and approved incision for c section is horizontal incision on lower abdomen then also horizontal incision on the uterus, but in emergency c section for a faster access to the abdomen ,a vertical incision is made from below the navel to suprapubic area ,then also a horizontal incision is made on the uterus. The vertical incision on uterus is rare because it may predispose the uterus to rupture during pregnancy and contractions. The horizontal incision on skin has some advantages:1-there's less post op pain 2-there's less tension on your incision 3-there would be less scar tissue 4-it would not be visible and you can wear bikkini 5-less adhesions will be formed within your abdominal cavity and less probablity of intestinal obstruction
But believe me that even in very emergent cases gynecologist prefer to make the horizontal skin incision because they can do it faster than any other incision.
