it starts off in a small spot than spreads to lmost cover the nail?what is it ?

You have a nail fungus. it's really hard to get rid of. The thing to do is put vicks vaporub on it and keep covered.
Doctor told me that this is better than any medicine for fungus.

Smoking and regular use of nail polish can cause yellow fingernails. Some home remedies that I have heard of is using Denture Tablets to soak your fingers in. Dissolve the tablet as it states on the package and then soak fingers in it for a few minutes and just repeat.
Also you can use Lemon Juice to soak your fingers in, just once your done soaking use some hand lotion to replenish moisture in your hands, and regular buffing can work as well, just not as fast.

The first thought that came to mind was lack of exposure to air, probably as a result of wearing nailpolish for too long. After reading your details, I am more inclined towards the fungus/bacteria path. Ask a doctor.

Sounds like a fungus. You might be able to stop it with an otc (over the counter) treatment that's specifically for that, but I believe that there's also a prescription that can be used as well. You might want to ask your general practitioner or podiatrist about that. I believe it's a "brush-on" treatment-you might ask your doctor about side- effects, or contraindications for using any anti-fungal medication on your nails.

It's a fungus. They make a pill for it.Talk to the local Pharmacist.It's advertised on TV. I think it's called Lamasill.
