First, symptoms are one thing, but its hard to say what it is without knowing what doctors have ruled out based on tests and studies.

Also, it would be useful to know what diseases you do have. Do you have diabetes? Neither 140 or 180 only 20 minutes after eating are diagnostic of diabetes.

Lupus would be my first thought. After that a variety of rheumatological and autoimmune diseases are possible. Possibly scleraderma with CREST syndrome. Find a good rheumatologist at a university hospital and see if they have any ideas. The other possibility is a neurological condition, such as MS. For that you should see a neurologist.

Also, don't forget the strong connection between mind and body. Living with chronic disease can be quite stressful. That stress may be worsening your symptoms. If I were you, I would also talk with a psychologist or psychiatrist to get some support in dealing with the emotional toll of living with a chronic disease. In addition, it seems you have developed a great deal of anxiety about your health (and perhaps, rightfully so). However, anxiety and depression can do two thing that would warrant treatment. They can cause existing pain to feel worse and they can cause a variety of problems with digestion, lethargy, and pain. Perhaps if you got evaluated for this some of these symptoms might feel better.
Some relaxation techniques may help also.

Lastly, a few things. As mentioned before, unless you are actually diabetic your blood sugars 20 minutes after eating sounds fine. Save your finger the pain of checking and stop checking if you are not diabetic. (Unless your doctor has instructed you too, of course.) If you are diabetic, you should be checking 2 hours after eating, or before eating (depending on your doctor's orders) not 20 minutes after.
Also, your urine will change color depending on what you eat and how much liquid you take in a day (not just water, but liquid in food and drinks.) Red, red-orange, and rust colors are bad for urine and may indicated disease. But orange (very dark yellow) may be just concentrated urine.

Anyway, good luck. I hope you find an answer soon. Again, if you haven't yet, check with a neurologist or rheumatologist. They may be able to help.

Ok, have they found protein in your urine? Have they done any fat biopsies, bone marrow biopsies? Reason I am asking is there is a blood disease out there called Amyloidosis, very rare. Many go undiagnosed until it is so late in the game. It is amyloids attacking your organs and/or central nervous system. Central nervous system can cause problems with BP, especially orthostatic hypertensive, can cause digestive problems as in not getting nutrients (malnourished) or diarrhea/constipation. Look on Mayo Clinic's website and read or amyloidosis.org. My father just passed away from this. For 9 months he was tested for gastro, heart, kidney, you name it. It wasn't till he went to a teaching hospital that they diagnosed him and he died a month later. I hope this is not it, but doctors don't know everything or every disease.

keep on going to different types of doctors that specialize in different things....when Montel Williams was younger he had to go to more than 20 doctors for one of them to know what was wrong... keep going to different docs and dont give up....and say a prayer for yourself every day...it WILL HELP. GOD BLESS YOU and may someone find out whats wrong and be able to help you...in the name of jesus, amen. :)
