Trust me when I say that that coy crap girls do when they like a guy is useless. Be honest and he'll respect you for it.

Well do you know the boy first? Is he a mate of your or just an acquaintance? It would be of a greater benefit if you knew the boy first and know what he is like first.
A boy would like to get asked in an environment he finds most comfortable, such as maybe a skatepark or a local beach, also if you are uncomfortable then there is no doubt that he will be to.
Boys do pick up on your body language so a casual approach would be best, and try not to fiddle to much with your hands or clothing, and if he says no, then the best thing to do is to laugh about it with him and therefore lighten the mood between the two of you.
There is always room for improvement in whatever you do so don't be discouraged if your first attempt does not go to plan. I'm not sure if any of this will be of any help, but good luck!!

Winter formal.... in January? Ok...
I like the simple easy way, no pressure.
Don't say the word date, that might give the guy the wrong impression like "oh great now i have to spend the whole night with her, have every dance with her blah blah blah"
instead, i'd just ask while you're at the dance
ask for a dance! "save the next song for me!"
i think having dates is really overrated plus, even i find myself being stuck with my date the whole night, i wanna have fun!
dance with loads of boys, not just one.

You could put a million post it notes on his windsheild with the message facing inside the vehicle so he just sees a bunch of stick notes... and then inside say something to the extent of "will you go to formal with me" or simply "formal?" blah blah blah
Make sure the outside of the car says something like Look inside! or something so he dosn't try just taking the sticky notes off...
it's time consuming but an impressive way to ask

Get a starbucks coffee cup and put a small slip of paper saying "go to the winter formal with me?"inside. *Dont get a transparent cup* Give it to him. When he tries to drink it and cant, he'll open the lid to see whats up. Then theres the slip!
