In the hospital I was told I need to fill out the paternity form only if I want to give my son his father's last name (I gave him my last name). But we filled the birth certificate form together with my partner (child's father), thinking, his name will appear on the b.c. The space was empty, and my partner is accusing me I deleted the information about him on purpose, just to hurt him. Of course it is not true, it was a mistake, but how can I prove it? I don't even think I should deal with such accusations. I brought the papers, so we can correct the birth certificate and even put my partner's last name, but he says I should have done it that time already.
I'm sorry, it was my mistake, we can correct it - he doesn't listen.

GO TO THE COURT HOUSE AND GET THIS FIXED. HIS NAME SHOULD BE THERE ALSO. CALL THE COURT HOUSE AND FIND OUT WHAT YOU NEED TO DO.

When my grandson was born, the father had to fill out the admission of paternity form in order to have his name on the birth certificate. If that form is not filled out, his name will not be on the certificate. Someone should have explained that more clearly to you both.

If your partner will accuse you of something like that, then not listen to your explanation, you got bigger problems than what it says on your child's birth certificate. Forget getting the paperwork corrected for a minute, and get yourself and your partner into couples counselling to address the lack of trust in your relationship.

so go and have a paternity test to see if he is the dad and then if he is then make a new birth certificate put his name on it... now if the partner wishes to have the child have his last name you will need to go to court and have it leagally changed.

Tell him to quit being a baby, you already apologized and are making the effort to fix the problem, tell him to get over it, I bet your son is more mature than his daddy.
