Short & to the point. If he wants a reason tell him, but don't drag it out, & don't say you want to be friends unless you really do...but if he is driving you crazy you probably don't want that.

Take full responsibility for the breakup. No matter how you feel, tell him he's a great guy, he's just not the guy for you. He's going to be hurt no matter what, but there is no reason to kick a guy while he's down. Be honest (just not brutally honest): let him know that you feel like your personalities just don't fit together and it isn't working anymore. You could even throw in that you're sure he'll find a great girl that can appreciate him more than you did just to boost his self-esteem a bit. Most importantly, do it in person. I know it's awkward and uncomfortable but it's just common courtesy. He'll probably be crushed when you do it, but you should at least have the courage to tell him to his face. Somewhere like a coffee-shop always works well (that's my personal favorite) because you're not stuck there too long (no food) and it's not uncomfortable to just leave when you're done. I agree, be firm. There is no easy way, when there was a point when you both felt the same way about each other. Some people dont get the hint its not working so just stop spending time together, get your self together and eventually he'll start looking for someone else to be with. Find someone who makes you feel good, and everything else will fall into play.

Honestly. Tell him you are not interested in him anymore and you are going to start dating others. Don't answer your phone when he calls and don't let him in when he comes over.Be very firm and consistent.He'll get the hint.
