Most piercers pierce a cartilidge with a 12 or 14. Do you already have yours done? If you got it at a real place, your piercing is probably in the 12-14 range. If you went to Claire's, you may have ruined your cartilidge anyway and it won't be an option (guns are bad! and they can shatter your cartilidge!). Yes, you can have them stretched. Do I recommend it? No. In all actuality, most places refuse to do it because it is so hard and painful. You may want to think about punching the skin to the size you want.
Before you do this, make sure to take into consideration a few things: your pain tolerance (stretching lobes can hurt, so cartilidge will not be fun), how easily your other piercings got infected, how careful you are with your ears (ie. do you wash your pillowcases enough, do you have long hair, etc.).
The last thing I wanted to bring up so that if you want to "gauge" to only a 12, do you realize that you'll just wear normal piercing jewelery? Plugs don't come that small (the smallest I've seen are 10s and you won't be able to tell it is a plug in your cartilidge).

you hsoudl have a dermal punch with a 12g.. if they do hem in that smallof size. or ust have it pierced with a 12g. i do not think you can stretch it liek you can with an earlobe as the skin is not fat and fleshy nor as elastic as the earlobe is... you will probably need to get it pierced with a 12g needle. or dermal punch.. bloody and painful.. and a long tiem to heal.. thats all i know. and i dont think the ear will ever heal.. but you shoudl just go to your piercer and talk to them about it.

your piercer can probably pierce your ear with a 12g needle and then put the plug in right away, but you won't be able to stretch it like you can a lobe, obviously. it will hurt, but not TOO bad. i think the hole will be too big to close up though..

counting on your point of tolerance for soreness i could say any gage is going to injury a minimum of somewhat. in case you relatively need to gage yours opt for it! it relatively is going to purely injury for somewhat after which you would be able to take exhilaration in it ")
