About 5 years ago, he lost his hair in the exact same spot. No doctor could explain why that happened and prescribed topical creams. The hair came back as peach fuzz. Just a couple of days ago, he noticed the knot. It is not a boil, it's under the skin and rock hard. What could this be?

I think that it is normal. Every one has a knot on their head but some peoples could be bigger than others. If it is hurting them then ask them to go to the docter and get it checked out.

Could be a lymph node. I had the same thing come up on the back of my head, right behind my ear. I had no other sign of infection, but my doctor put me on a long low dose antibiotic(I took it for a month) and it went away.

Could be a tumor have him go back and get it checked to be on the safe side.

Could just be a sebacious cyst - they're usually harmless, no need to have them removed or drained.

It could be cancer because that is usually how you get cancer is by a knot like that.
