When you dont brush your teeth you get that yucky rough feeling on your teeth. People probably noticed that feeling and liked it when their teeth felt smooth. Initially people cleaned thier teeth with sticks

Also from what i understand even the most uncivilized culture has tools for taking care of their teeth, so it probably goes all the way back to when we discovered fire.
