How to get back together with my ex-girlfriend? (I'm already fairly sure she wants to get back together)?

Just asked if she sounds like she wants to get back together with me, and forgot to mention I want to get back together as well. I broke up with almost a full year ago because I was going through a life crisis involving drugs and a nervous breakdown. But we recently started hanging out again, and I'm pretty sure she wants to get back together at this point. We've both dated a lot of different people since we broke up, but have both been let down in those relationships. Any advice on how to go about this?

We dated for about 2 years, and were very serious about getting married before we broke up.

If the two of you dated for two years, then the ice is broken, so to speak - no reason to be bashful about asking her out, right?
