Lack of Circulation?

My brother is on his way to the docters right now because his two middle toes are completly white and he can't feel a thing with them, it's been like this for like 20minutes and he's a nurse(a new one) so ovisally all these bad thoughts are flowing to his head what could be wrong. What could be wrong? Honestly! I want the truth. I'm worried.

possibly diabetic foot (if he's diabetic) or other forms of ischemia or blockage of the blood vessels. if he's overweight then his chances of that happening go up
