The most common cause is tension in the muscles of the shoulders, neck and head, which includes facial muscles. It is believed that migraine is a tension headache which has gotten out of control. Stress is the greatest cause, sometimes when we have a fever, or bad head cold, a headache can occur especially when our sinuses are swollen and congested. Sinus headaches hurt in the bones of the face and behind the eyes. Migraines tend to be on one side of the head or the other, though it can escalate to the top of the head as well. Migraine headaches can be quite severe.

Headaches are one of the most common complaints. They are also one of the most difficult problems to correctly diagnose. The National Headache foundation linked below has some very good information. Headaches have a wide variety of causes, ranging from eye strain, lack of sleep, stress, sinusitis, migraine and tension to life-threatening conditions such as encephalitis, meningitis, cerebral aneurysms and brain hemorrhage.

this isn't a joke, but my headaches are caused from my jaw being out of line. the stress and tension of my jaw causes my head to be sore...
also at the minute, clenching teeth in my sleep
but it can be caused by loads of other things - dehydration, sunlight, lack of sleep, etc etc!
