I am a GP - The medical term for an itchy bum is 'pruritus ani'. There are various causes for this and it is not just piles that can cause this. The problem with many of the preparations that you buy from the chemist is that you can develop an allergy to the constituents of the products which makes the itch cycle worse. Other causes of this itch could be a skin infection, sweat rash, infected gland or 'worms'. Make an appointment to see your GP a.s.a.p. If you have had rectal bleeding then you must see your GP in the morning.

i had a similar type problem recently . I attended my local GPs Surgery. The Doctor examined me, explained that for the time being he would be able to push the piles back up. Further more, he gave me a prescripton for a course of medication that I must see the course out (as you must with anti biotics) and informed me that once daily I would have to find assistance of the pushing of the piles up in a similar type manner of that which he done.
Having collected my prescripton and returned home forthwith, I explained to my wife that she would have to assist me in the pushing back up of the piles once daily in addition to me taking the prescribed medication.
Unfortunatley, on the first occasion that this was attempted it did not run smoothly, so to speak (excuse the pun). as i noted that my wife was using the completley wrong method which caused me some considerable discomfort. I informed her almost immdiatley "NO NO, Woman! You're getting it all wrong! The Dr had both hands against the wall when pushing the piles back up!"

See your GP is the first advice, Germaloids ointment is very good ( it contains an anaesthetic ) Also take "Fybogel" it's a fibre supplement (orange flavour is best) twice a day (one morn one night) I used to suffer really badly but that stuff changed my life completely, i now only take one now and again. I totally understand what you are going through so my advice is genuine good luck.

You need more fibre in your diet, also try not to strain on the loo andkeep the area extra clean and dry. warm baths can help ease it too. Id also suggest seeing your dr if it persists as there are things they can do.
