Usually with a lot of swearing and exasperated sighs. But it is easier if you have someone help you. One person holds the cover open while the other stuffs the comforter in.

Yes, you can. You can also put a duvet over an old comforter. Putting a Duvet on can be a pain. So when you first get the comforter to reach the bottom of the Duvet, pin the Duvet to the comforter with safety pins on the corners while you adjust the comforter evenly.

I lay my duvet cover on my bed, inside out and work from the bottom corners to the top, turning the duvet right side out as I work.
I also stitched in the inside corners of the duvet long ties that I secure the corners of the comforter to in order to keep the comforter from slipping around inside the duvet.

Oh...this is going to be difficult to explain...its more of a visual thing...Here I go:
Grab one corner of the comforter and put it in to the corner of the duvet cover. Do the same with the opposite corner. Then shove the rest of the comforter in, but leave the other two corners out.
Now grab the two corners of the comforter you left out and put them in the empty corners of the duvet cover. Hold on two these corners and shake the entire duvet and comforter. By shaking it, you get everthing into place and tah dah!!! Your comforter is in your duvet cover!! Hope I didn't confuse the heck out of you....

You don't........ you put a duvet cover on a duvet!!!!!!!!!!
But seriously......... put the bottom corners of the duvet in the bottom corners of the cover. Have a firm grip on the outside of these corners and shake. You can even use clothes pegs to hold the bottom corners until the cover is 100% lined up.

I put the bottom corners in place and then safety pin them just long enough to get the rest of it lined up right and then buttoned up.
It's not really my favorite choice of bedcovering, because it invariably feels so heavy. They're almost never cheaper than a different comforter, either.

I usually stuff it all in there, grab the corners and do the big up/down swing to get that blanket all spread out inside. After that, you just button it up.

I personally shove it in and then stand on my bed holding the top two corners so it spreads out evenly. Sometimes, i even crawl in and get it straight.

put the back corners in first ,or fold the quilt in half and slide it in like a pillow in a pillow case.
