what are the documentes to be obtained when upadating the patient insurance info at provider?

this question is regards to Us healthcare system.

I am a medical office manager,and to update a patients insurance information we just need the patient's new insurance card(s).....there is no other documents needed. If a new patient, then a history form, a form giving address, mailing address, phone number, photo ID ..or drivers license and of course the insurance cards....
