I don't know about the second question, but for the first and last questions, I would say that yes they can travel from Malaysia to Singapore then back to China. There are two ways to do that.
One way is to purchase a return ticket from Shanghai to Kuala Lumpur on Singapore Airlines with a stopover in the outbound sector. This way, they can fly from Shanghai to Singapore, where they enjoy the stopover, then fly later to Kuala Lumpur. In the return flight they fly KL-Singapore-Shanghai with just a quick connection in Singapore. Unfortunately, this cannot be done with Xiamen because Singapore Airlines done not serve this route.
The second way is to come to Singapore in any way they like, then purchase one-way ticket from Singapore to Shanghai or Xiamen on Malaysia Airlines with stopover in KL. This way, they fly to KL where they enjoy the stopover, then fly directly on Malaysia Airlines to their respective cities.
They might as well do it in series of oneway tickets, but this might be expensive. These are the airlines serving each sector:
Shanghai-Singapore
China Eastern Airlines, Garuda Indonesia and Singapore Airlines
Xiamen-Singapore
Air China, Silk Air and Xiamen Airlines
Singapore-Kuala Lumpur
Japan Airlines International, Malaysia Airlines, Pakistan International Airlines, Singapore Airlines and SriLankan Airlines
Kuala Lumpur-Shanghai
China Eastern Airlines and Malaysia Airlines
Kuala Lumpur-Xiamen
Malaysia Airlines and Xiamen Airlines

In the Singapore Straits Times, one employee complained that there have been a few miscommunications among the Chinese nationals and Chinese Singaporeans...i suppose it is the accessory/ pronunciation/ distinct tones... just lately, there are murders regarding China nationals right here in singapore too...

Well, I just visited Hong Kong, then Singapore, and then went diving in Malaysia before returning to Singapore and flying back to my home country (Netherlands).
So for my case:
1) yes
2) no
3) sure
but how this will pan out for mainland China people? Hmmm... I would probably contact the relevant embassy before attempting this.

as far as i know, they can go to malaysia from singapore with a visa. the problem is they cannot go back to singapore after they exit from singapore as their visa is valid for single entry. (this is the normal case unless they apply multiple entry). of course they can fly from malaysia to china. but the nearest airport is about 3.5 hours from singapore (KLIA airport).

Q2. Yes, it is advisable. I think visa on arrival is available, but it'll be more expensive and risky, if your friend is already confirmed to come to M'sia, just get the visa prior to save hassle.
