You'll have to look into your policy to be sure or ask BCBS.
I know My simple Card benefit definitely includes lap
band surgery.

Here is the website for BCBS of Michigan Member services. You should be able to plug in your contract # and get what your benefits are and what is covered under your contract.

Probably not, but you need to call them directly to be sure. Many of what they deem to be "cosmetic" or "elective" surgeries are not covered, unless the employer has a specific rider on the plan to allow them. Because weight loss surgeries have a high risk of complications and are not considered lifesaving (the way the removal of a diseased appendix would be), they're often considered elective.
