Don't sweat the petty things and don't pet the sweaty things! Ha ha. Seriously... twice in a year is nothing to worry about. If you see the dog do it.... scold him so he knows it was not the right thing to do. But otherwize... don't make a big thing out of it. Consequently..... this may be unrelated.... but I had a friend in high school who had a dog that would only pee in the house once in a while. Eventually she realized it was only when she was menstruating. The dog was an un-neutered male and his instinct was to "mark" over the spot to cover the scent of "a female in season". No delicate way to put it. This probably isn't your situation, but worth mentioning just in case..... And why do people want to rub the dog's nose in it? How silly. That only confuses them. I hope those people never have to potty train a child!

~Was she locked in? If she had no way to get out, she had to do what she had to do.
~Old pets get senile like old people do. She could be getting confused.
~Has something changed? Sometimes new furniture, or cleaning carpets, furniture or drapes will cause pets to want to "mark" their territories. Sometimes change makes them feel insecure.
~Is she afraid of something that may have been in the yard where she goes? A wandering cat (or other animal), a loud noise, or someone in a neighbor's yard may have spooked her.
Because it's not an ongoing problem, I don't think she's sick. (changes in bathroom habits sometimes means the pet is ill)
I think she is reacting to something. Watch her close and see if you can identify the cause.

First I would good a mattress protector - unfortunately the plastic covers that the put on kids bed who still wet the bed....Yes I had to do it....so I know how embarrassing this could be. Second something is laking in her daily activities.....when she was doing it what were you doing. Have you walked her or played with her. She is probably showing you that she is not happy with what is going on. It took me taking my puppies for a walk or playing with them for awhile when I get home. I would pet them and showed them love when I got home of course but the activities helped.

Take your dog outside to pee. It's not a major problem if this is only the second time in 1 year

I would get a mattress cover. Then limit liquids in the evening and take the puppy out before going to bed. You may also want to consider crate training.

You should Rub your dog's nose in it & tell him or her that was bad believe me mine did it once & has never done it again after I did that 2 her.Plus spray some vinegar on your bed they hate the smell of this but it kills the odor you use 4c of hot water & 2 Tbsp of vinegar put inside a spray bottle it does wonders believe me your problem will be solved I know mine is.Good Luck

Keep pet in the bathroom,for the rest of the day,with food and water of course. pet wont like it but every time pet pees,pet go to the bathroom the whole day...

Well, it's not a big deal. But, it's not a good thing. If your dog does it again in the next day or two, I would take him/her to the vet. She/He could have an infection.

Maybe shes mad at you.
Have you not been at home as much? If she knows shes doing something bad the only thing i can think of is to make her sleep on the floor.
