It sounds like he probably has a bowel obstruction. A bowel obstruction can be caused by a number of things. The bowel may be inflammed, there may be a defect in the colon (like a narrowing or a protrusion), the colon may be sluggish ( not moving waste thru at the proper rate),there may be to much waste in the colon and not allowing things to pass thru.
If he has a bowel obstruction, it is usually painful/ with the added pressure of having stool that can't move and gas building up. It can cause the person to have watery stools( as water is the only thing that can get by the obstruction, it can also cause nausea and vomitting( in an attempt to get the extra contents out), in some cases people have been known to vomit feces.
Treatments: stomach pumping( to relieve pressure/ remove any excess contents in the upper colon), enemas to get the bowels moving, giving meds that cause diaherra( if the colon is sluggish this will jump start it), and if severe they do surgery to remove any damaged colon or to relieve twisted colons, or to prevent the colon from bursting.

The hospital will take care of him. Probably give him a rectal exam and then go from there. Might be a blockage from constipation and like someone else said pain medications will constipate you.

He may just need fiber. At 19, I wouldn't expect too much to be wrong. No worries - they will figure it out quickly.
