You need to buy a scoopable litter and clean the box daily. The urine has an ammonia smell - which can make people sick but I don't know about passing out. It's a daily chore - something not fun - but neither is getting sick.

Yes, it can smell very bad if it isn't cleaned out enough or if kitty has bad urine smell or stinky poo.

What brand are you using? Is it bad before kitty uses it or after? How often do you change it?

I use Fresh Step and it smells pleasant as pleasant as it can be. It has a "Fresh" smell to it, hence the name Fresh Step. I change it every day. I keep mine in the laundry room. Change it daily to keep it fresh as it can be. Mine is the scoop-able kind, easier to clean out.
