what all does a pregnant woman that is switching her ob/gyn need to provide her new doctor with? my wife had alot of problems with her last pregnancy and delivery, so shw wants to ake sure her new doctor has all her information.

Just go to the new OB and sign a paper that gives them access to all of your medical records and they will take care of it from there.

Talk to the new doctor. The old Dr will usually have their records sent to the new one at the request of the new Dr, which is standard procedure. But ask the new doctor what they normally do in this situation just to be sure. You can always request the old Dr to send the info to the new one if all else fails.

I have to change all the time as I am performing in a show that tours. she must have the last date of ovulation and period, the expected date of delivery and how many weeks or months the last doctor has stated. This is so important because if he doesn't have all the right information the baby can be induced too early or too late ... harming your baby. Also take any blood tests you have had done. If possible get a photocopy of the information the other doctor has written about you

her new ob will have her sign a release of information form and send that to her previous md who must release the forms to the doctor..or she can go to the old doctors herself and request her records as they legally belong to your wife
