For labour:

Your birth plan
Dressing gown
Slippers
Socks
An old nightdress to wear in labour
Massage oils or lotions
Hair band
Lip moisturiser
Snacks and drinks
Watch with a second hand to time contractions
Camera and film/tape
Relaxation materials: books, magazines, games, music, candles
Pictures of someone or something you love (the inspiration you may need to see you through to the end)
Tens pain relief machine if you are planning to use one
Toiletries

For your birth partner:
Change of clothes
Snacks

After the birth:
Going-home outfit
Breastfeeding bras
Breast pads
Sanitary towels
Nightgown
Toiletries
Address book, plus lots of change or a prepaid phone card for all the calls you may want to make. You may find you cannot use your mobile phone in the hospital building

For baby:
Infant car seat
One outfit for the trip home (all-in-one stretchy outfits are easiest)
Baby blanket (a warm one if weather is cold)
Nappies
One pair of socks or booties
Hat
Jacket or snowsuit for winter

Once you've got your hospital bag organised, remind yourself of the first signs of labour so you know when it's the real thing

Extra clothes, loungewear (sweats and a Long sleeved Tshirt for winter), sanitary pads, a book, an outfit for baby and an overnight HBA bag (shampoo, toothbrush etc)

Many hospitals let you go home the next day after birth if all is OK and I loved doing that. I was so uncomfortable not being in my own bed and home.

I had a bag for labour/delivery, and a separate one for recovery/staying in the hospital.
You will probably just wear the hospital gown while you're there... but you might want to take some PJ bottoms. Definately slippers (hospitals are filthy). Clothes to come home in. Nursing bra/breast pads, maxi pads (the hospital ones are huge)
Of course things for baby... wipes, diapers, coming-home-outfit, blanket.

An outfit for you to wear in the hospital-unless you like just the hospital gown.
Socks for you
An outfit for when you leave for you
Any and all toiletries that will make you comfortable like lotion, lipgloss, shampoo, conditioner, body soap
An outfit for the baby to go home in.
A blanket for the baby

Those are the things you need for sure but just remeber it's about your comfort and conveinance and others can go fetch you something if you need it later because you will be recovering.
GOOD LUCK
