Depends on what state you are in, the average age and health condition of the employees, and what kind of coverage you plan on purchasing. You'll have to get a hard quote to stick.
Regarding liability, if you seat less than 10 people, and have NO friers, then it should run you about $1500 a year assuming gross sales around $100,000, auditable. You'll have to contact an independent agent near you for a specific quote.

It could vary so widely- cause ins costs are based on the age of employees, what kind of coverage you offer (ppo vs hmo), male or female, marital status, pre-existing conditions, all kinds of things. And if you haven't hired anyone yet, obviously you wouldn't know at this point what this info is.
