The BoM includes over 17 CHAPTERS of plagerized material from the Bible.
Isaiah 2-14 & 2 Nephi 12-24
Isaiah 48-49 & 1 Nephi 20-21
Isaiah 50-51 & 2 Nephi 7 &8
Isiah 53 & Mosiah 14
Not only that, but the BoM contains verse quoted from the King James Bible WITH THE WORDS ITALICIZED. The italicized words were only added into KJV by translators in the 16th & 17th Century...
...not only that, but the Bible chapters ripped off are written in King James English style...even though the BoM was written 600-500 BC.
So, yes, it was plagerized. Joseph smith just needed some help boosting the content of the Book of Mormon.

It was explained to me that it is individuals reading out loud portions of the OT.
My question is why did the Mormon writers reading from the original Hebrew text make the same translation mistakes as the translators of the KJV did?

that statement has a debateable answer depending on which mormon you ask.
these are the two options.
Yes, He copied some of the old testament prophets to save time,
NO, what he transcribed was done by the POWER of GOD proving that God has power to Keep scriptures pure and undefiled.
i believe most mormons think it is #2

The people in the Book of Mormon had a version of the Old Testament with them, and Nephi in particular wrote about how he read from Isaiah to his brothers that just didn't understand it.
It's not something we hide from, if you read the chapter headings of these "Isaiah chapters" it says "compare to Isaiah..."

Smith went and "corrected" parts of the bible that he claims that god told him were wrong. Actually, he just changed parts that conflicted with the stories he had told.
