To the best of my knowledge when i was in flight training every time there was mechanical failure (something as simple as defective guages to full blown engine failure)whether it kept a plane from flying or not was red tagged and logged in a book.Then god forbid that plane should crash that log book is there to prove any defect had been repaired or if there was a mechanical failure listed as cause of accicdent you could tell if it was a recurring problem or something new.

Red Flag is a series of major exercises at Nellis AFB using many types of aircraft in the air to air and air to ground mode. It aims at the total integration of ops and command and control with all the players. Often involves special operations forces as well as aircraft from other countries

I am comming at this from Mechanic"s exp, aware of Red flag at
Nellis/ When you work Flight line,you will document and work Air Craft according to Write ups in this book,also mechanic will place a Red X when Air Craft is to be grounded,until "problem"
is cleared according to Tech.Order Para.and step and operational check out,any Mechanic can place a red X--but only
selected few can clear Red X.As to color Red,it can be used to stop A/C or personel at the hold short of runway,should radio malfunction, When I worked for C.R.I.&P Railroad used Red Flag.Conclusion when you see Red Flag,stop what you are doing.When something has a Red Flag It has several sets of eyes on it
