You need permission from the father. Also, you will need a passport for her and all of her immunization shots up to date. If she is under 6 months, she will not be permitted to leave the country until she has had all her shots up until that point. Thank you and good luck.

A U.S. citizen requires a passport to leave the U.S. Depending on where you are going, she may also need a visa for that country, even if you, yourself are a citizen of the country you are going to. You will need an official birth certificate and at laest two passport photos. There is a fee and the process can take seveal weeks. The fastest way to acquire a passport is to go to a passport office, located in several large citties. There is one in New York, Los Angeles, Philadelphia and Washinton D.C. There are probably others.

1. fill out application for us passport
2. present proof of us citizenship (birth certificate, naturalization certificate, record of citizenship)
3. present proof of identity (picture id)

A passport and probably a Visa depending on what country you are taking her too.
You may also have to get permission from Home Land Security to leave and return .
