You could ask the doctor for clarification and then you'd know for sure, but what "I rule out cancer" means is that he does not believe the issue being addressed is cancer, but apparently doesn't know what it is.

You are somebody you know are suffering from an ailment and went to a doctor and he has examined and said to you that he rules out the ailment to be a cancer in orther words he very stongly believes that the ailment is not cancer but something else.

It means that he has determined that you don't have cancer. To rule out means to disprove or eliminate a possibility.
