You'll need to have a receipt from them. Most churches send out an end-of-year statement to their members. They need your name and address for that, not your social security number. They don't report your contributions to the IRS, so should have no need for your number.

You'll need the receipt if you get audited.

If you donated a vehicle to the church and you want a tax deduction for it, unfortunately you do need to provide them with your Social Security number.

But generally it is not okay to give a church your income information, date of birth, mother's maiden name, drivers license ID, or especially your Social Security Number. Bad idea.

If you gave the church any other donation, then request a receipt from them but you don't provide them with your SSN.

The new tax rules explicitly require proof of donations if your donations are over $250, and a summary is preferred for the whole year. That menas the church will need to match donations to donors to give you a year-end statement, which has your name, address, and probably your SSN.

You do not HAVE to give your SSN to the church, like you would a bank that pays interest.

You don't even need a receipt if you are not itemizing and deducting your contributions.

They do not need all of your information. All they need to know is you name, mailing address that's it. If you have given money to the church they should give you a receipt that's all.

Yes, I do. Otherwise you won't be able to write off your contributions. You MUST have something from the church stating how much you contributed in order to get your deduction.
