he doesn't believe me because everytime I come running back, and I have told him so many times that I am seriously going to leave him if he keeps it up. I know he cares, he just doesnt belive me because no matter what I say I always come back.

First things first, be confident that you are ready to leave him, and be serious about leaving. Then, let him know that he needs to ship up or ship out. If he cares, he will take you seriously and speak to you about this.
Be an adult, that means no yelling, screaming, CRYING, none of that. Write a list and be prepared to let him know exactly whats on your mind, don't attack him verbally, be well thought out.
One thing I know, is that people always want what they can not have, with that said, if you do leave him, be sure to find a rebound guy just so knows what he's missing and you might find that he will come CRAWLING back!

Warn him!!! Let him know that now u'd have enough of him! Personnaly i don't think people change, neither do i think that people should be forced to change!
If he really cares for u, he'll do it! The other way, just go!! he don't care!!!!!!!
If he does not take u seriously then do what u should have done since long!
