The World Health Organization is working toward eradicating polio throughout the world. In 1988, 355,000 cases of polio in 125 countries were reported. By the end of 2004, there were just 1,255 cases.Six countries (Afghanistan, Egypt, India, Niger, Nigeria, and Pakistan) still have polio circulating, and the virus could be introduced to other countries. So to answer your question no it has not been eradicated completely

No, but it is rare in the western world due to excellent medical treatment.

Poliomyelitis, often called polio or infantile paralysis, is a virally induced infectious disease which spreads via the fecal-oral route. It may proceed to the blood stream and into the central nervous system causing muscle weakness and often paralysis. An ancient disease, it was first recognized as a medical entity by Jakob Heine in 1840. Vaccination and eradication efforts led by the World Health Organization and The Rotary Foundation of Rotary International are credited with the reduction of the number of annual diagnosed cases from the hundreds of thousands to around a thousand.

Unfortunately Polio is still at large.

You can get information and the latest counts with breakdown by country on http://www.polioeradication.org/casecount.asp

The global count for 2006 was 1902 cases.

At present Nigeria is leading the list followed by India.

Until last year they thought they had it under control, that is until a community of Amish started to have an outbreak of Polio, because they don't believe in vaccination, they urged them to take their kids and get them vaccinated, some did ...some other didn't. The Health Department is trying to make them understand the importance of getting the vaccine. I am not sure if they achieved the goal but ...that helped them realized how important is to educate people in all communities.
