#1 Trauma, Otherwise in no particular order- mono, sepsis (streptococci most common folowed by staph., gram negative organisms and anaerobes), leukemia, and very rarely a pancreatic psuedocyst dissecting into the spleen. Vascular aneurysms and hemangiomas might as well.

Not sure, but at a guess I would say:
Internal Hemorrhage - bleeding
Infection
Aggressive Cancer
Toxic Substance
Trauma that is not known about, but did occur, such as if the person was hurt whilst unconscious.
