Put some padding around the top, maybe some tissue? The clicking comes from the plastic hitting other pieces of plastic, if you can pad that collision it will be nearly silent.
Or you can just use a twist top pen.

Wrap it in something to muffle the sound? I don't think anyone makes silent pens (unless there are some luxury non-click models out there)

Um, I do this each and all the time while i'm working. while i'm on my rounds on the ICU, I examine my sufferers necessary signs and indications and then write down something. while i won't be able to make your strategies up what's incorrect with my sufferers (i understand, i'm a awful nurse), I do this so i will think of.

i really dont think its possible everything makes a noise even if its about 1 decibel ( thats about a pin dropping) so try taking out a few parts, experiment a little! rofl , lmao, lol !l!

Take it apart and put some vaseline in it. That'll quiet it down, but won't make it silent.
