The problem as I see it is not in having the tattoo done but in the tattoo itself - ugly, cheap and nasty things. I do not understand why people wish to brand their own hides in this way.

There is not problems with a physical. But if you have a new tattoo and go in for a massage, make sure the therapist knows. If the tattoo is new and not completely healed the lotion can lighten the ink.

I don't believe it would interfer with a physical exam. The doctor or nurse might ask you if you had the tattoo done at a reputable place that used sterilized needles. If not, he might want to have you get a blood test to see if you got any bad diseases from it.

It won't cause any difficulties during a physical exam. Nor will it be an issue for any other diagnostic tests, including radiologies and blood work. You should probably not get your tattoo right before your scheduled exam just because it will be a healing wound, but it certainly won't cause any issues once it's healed.

The only medical concern is that unsanitary needle used could lead to hepatitis. Also if tattoo is recent, you cannot donate blood

No, having a tattoo will in no way affect your physical with your doctor.
Unless you have Hepatitis or something, but....I don't think that's what you meant.
