When I had my son I gave him my last name. There was no father on his birth certificate also. I got married also. So what we did was my husband filled out paper work called " Voluntary acknowledgement of paternity." We mailed that into to Department of Public Health and Environment. We had to send $20.00 in with it. On the sheet there was an option to change the name also.
We got all this information through our Record department in our county. If you need any help go to your record department (the place where you can get birth certificates and death certificates) they can lead you to the right place!

Go where the birth certificates are issued in your town and take your husband and marriage application and tell them you want to fill out an affidavit to have her name changed. You will probably have to pay but it is worth it. When you get the new birth certificate you will have to take that to the SS office and have her name changed on that as well other wise you will have to use the name on her current SS card when you claim her on your taxes.

All you have to do is go down to the courthouse w/ your husband and have the birth certificate changed. Some courts may charge a small fee, but it's as simple as that. I did this w/ my daughter's dad. I have since regretted it, but it can be done!

x-(To hell with the father! I have my mother's last name! I belief that the child should be named after the mother! Also the religion should be from the mother also! What did the father do? All he did was make love. He was not pregnant for 9 months. He wasnt the one sick all over the place. He wasnt the one who gave birth and suffered the pain. If he leaves you right now it is your responsibility to care for the child. If the father abandons his child (while mother and father together) he wont go to jail. Have charged of child neglect! But if the mother leave the child she can be put to jail! The father has no resonsibilites at all. The child should have the mother's name!

You have to go to the Social Security board and have her name changed, but first you have to get the dads name on the birth certificate first. I pretty sure you do that at the court house, but they didnt help you right? Humm...I would call them again because they are the only ones that can help you with that.

go to your local court house and they will give u the info on how to change the name plus they will be able to help u get a new birth certificate from the hospital with the real father name on it

Within a certain period of time, you can file an amended birth certificate. It may be over the time limit,, in which case you would have to do it in a court.

You have to go to a social security office. Present your driver's license, your ss card, your child's birth certificate and your marriage certificate. They also have forms at the office that you need to fill out.

Go to Juvenile court or wherever your local child support bureau is. He will have to sign papers acknowledging paternity. Fairly simple. To make things easier with your daughter, go ahead and just start using her new name.

he has to establish paternity... i live in Texas and i know in Texas you do that through Texas Attorney Generals office but if you don't live here I'm not sure were you get that done if it's not at the time of birth
