My guess is that they'd put a little wrist band on you that says "John Doe" just as if your name was XXX and they'd just treat it like any other name, they might question you at first but a proof of ID would settel it.

When you're admitted into a hospital, you are processed due to information about you through your chart or id you carry with you. You will also be catogorized as Doe, John.... and have information about your medical conditions in the record.

Initially they would probably put you in a double room with John Smith.If they finally found out that your actual name was not John Doe they would classify you as "unknown male" and put you on the list for shelter placement!

In the age of instant info with the inter-net etc that dilemma could easily be sorted out. A person has many identifiers,not just a wallet ID. Like your car tag,registration ,insurance,store receipts to name a few.
