you could have a bronchial infection or some minor pleurisy (inflamation of lining between lungs and ribs)...you may have even strained some muscles...have you been coughing a lot? Picked up anything heavy or moved wrong?
there are several muscle groups involved in this area...there are lots of ways to get this kinds of pain. Bronchial infection, asthma, or body strain are the most common.

Sound like you pulled a rhomboid muscle it is the one just below and between the shoulder blade and it hurts like nobodies business. There is not much you can really do for it but rest and let the muscle heal itself. Take it easy and lay on a heating pad for few minutes every hour

you should go to the doctor, it may be nothing but i had this and just thought it was smoking till i got a fever and ended up in intensive care with plurasy. get it checked out its not worth leaving!!
