hi try this

http://www.direct.gov.uk/DisabledPeople/HomeAndHousingOptions/YourHome/YourHomeArticles/fs/en?CONTENT_ID=4000642&chk=sI4nRl

website says:

"Disabled Facilities Grant is a local council grant to help towards the cost of adapting your home to enable you to continue to live there. A grant is paid when the council considers that changes are necessary to meet your needs - and is happy that the work is reasonable and practical"

another page on how to apply:

http://www.direct.gov.uk/DisabledPeople/HomeAndHousingOptions/YourHome/YourHomeArticles/fs/en?CONTENT_ID=10018283&chk=y4L9fn

also other stuff that might help

you might be able to get carers allowance, reduced council tax and other stuff that might make life a bit easier

but i think the best thing to do would be to ring socail serivices as they know how to help the best, with practical things and know all the contact numbers they have a list of charites that you might be able to apply for funding from i know socail serivices sounds like a pain but i reckon they could help out try giving them a ring
