First off, in order to shrink a dog in size there is inbreeding and line breeding. There are formula's created just for this purpose. Using genetics and grids helps breeder predict what kind of pups will be produced by pairing off dogs. Selective breeding. Breeding unrelated dogs tends to increase the size.

Sorry, but I don't think that you can find a breeder that doesn't do that.

As far a health check all puppies should have had their first shots and be examined by a Vet., that is just what competent breeders do.

Back yard breeders don't pay any attention to the breed standards nor do they cull their litters. And produce a littler of pups that are nothing more than pet quality. Which there is nothing wrong with a pet quality dog but if breed and breed and breed will produce lesser and lesser quality of dog.

Sounds like you need to get on the phone and call around. Ask Vet's in your area for breeders, all breeders need to have a vet. Log in the the AKC site and check their Stud Book.

Contact your Kennel Club as they will have a list of there members which are reputable breeders. There is a publication here in Canada called "Dogs in Canada" that lists all breeders for all breeds, perhaps there is such a publication where you are?

They should be able to point you towards a suitable breeder.
There is also a link to required and recommended health testing.
