If your New York registration runs out, you run the risk of being ticketed for driving a non registered vehicle.
Per NC's web page:
How do I title & register a vehicle?
In the case of new residents, you must register your motor vehicle at the expiration of the time granted by reciprocity agreement between North Carolina and your prior state of residence (usually 30 days) or when gainful employment is accepted, whichever occurs first.
So, if you are working, or there over 30 days, you need to change your registration. At the end of 4 months if you move back to New York, you will have to change it again.

No, if you are not registering the car in NC then you have to get a NYS inspection. Even though it expires, you dont have to get it done until you come back to NY.
