Not if they are younger than 12 weeks old. If they are over 12 weeks old then , yes they will need one. Just have your vet give you a shipping health certificate and they will give you the right paper work , according to her age and vaccination history.

In Canada and most countries yes. Here though, if the flight is within the same province and the puppy is to young for the shot, under 5 months, you don't have to no. But you have to prove the other shots and the age.
If it is an international flight even with papers, there is often a quarantine time.

yes, all vaccinations need to be current and the dog needs to have an exam by a vet before travel. Call your airline for exact documents etc. needed before travelling and then make sure you call your vet so you make enough time to get all the exams and documents completed

Any animal shipped needs to be up to date on their shots and be cleared from their vet.

number 1: they are traveling and they ask u for the paperwork of the rabies.
number 2: your dog wont be able to travel
