That's a lot of responsibility to put on another human being and it appears to be from someone with little life experience who wouldn't know the burdensome nature of the note.

Oh, right, I just read your update. He's playing and you have a really good friendship with him. If you're trying to "get more". Don't become confused by a playful little fill-in-the-blanks note.
On the other hand, he could be one of these wind-up merchants who knows you fancy him and gets his jollies off of playing with your head knowing you will wonder about the answers.
If this is the case, stay away. Go for a fella who has enough self-respect for himself and you to only speak honesty and the truth as he knows it.

Make a copy and give it to his school counselor. This guy has issues. Get his family to get him help before you run for the door. But you do need to get out of this sounds a bit alarming. Let your parents know that he's obsessed with you. if you are old enough to be sexually active your old enough to let your parents read this.
