There is nothing in the Scriptures to indicate that Satan and his demons can read our minds or thoughts. Rather, the implication of the Scriptures is that only God can read the mind of man: “Mere man sees what appears to the eyes; but as for Jehovah, he sees what the heart is.” Further we are told regarding God: “There is not a creation that is not manifest to his sight, but all things are naked and openly exposed to the eyes of him with whom we have an accounting.”—1 Sam. 16:7; Heb. 4:13.
However, most likely the demons can, because of higher intelligence and better powers of observation, note what is in the mind of man because of his actions and facial expressions. Thus when a man goes contrary to Matthew 5:28 ("But I say to YOU that everyone that keeps on looking at a woman so as to have a passion for her has already committed adultery with her in his heart") it may be apparent on his face, and so one could tell what is in his mind even though being unable to read it. So they may take advantage of weaknesses that one makes evident and tempt him to sin.

Because you know your own weaknesses. When you know your own weaknesses, you know the best way to sabotage yourself.
Of course, it's convenient to blame these shortcomings on Satan, and that brings about questions as, "How did Satan know to cast doubt on my relationship and inflame my jealousy?" In actuality, you were able to cast doubt on your relationship and inflame your jealousy, but it's harder for people to admit to themselves that they may be at fault.
If you acknowledge that most problems that befall you are the result of either your action or your inaction, then you will realize that Satan has no hold on you. You are responsible for your well-being and blaming your downfalls on Satan just makes you a weaker person as you have no motivation to improve yourself.
Edit: That's good that you don't blame Satan for your weakness. But why do you think Satan is tempting you? Everyone has desires, and they're not all flowers and sugar. How you deal with that desire determines what kind of person you are. If you give in, then it's not Satan's fault. You gave into that temptation. Where did that temptation come from? Within ourselves.

In Judeo-Christianity, Satan is the heavenly prosecuting attorney. Many of the references to Satan in the new testament (such as Yeshua's famous "Get behind me, Satan") are using the word in the sense of... a word. "Satan" actually means an enemy or opposer.
Satan, still in Judeo-Christianity, knows your weaknesses because that's how he was made. He puts your love of God to the test so God can accept those that truly love Him. Satan is blamed for many horrible things in the world when God is actually the root of all good and evil.
Isaiah 45:6-7:
"...I am the LORD and there is none else. I form the light and create darkness. I make peace an create evil. I the LORD do all these things." (KJV)
Job 9:22-23:
"...[God] destroys both the blameless and the wicked. When a scourge brings sudden death, he mocks the despair of the innocent." (i.e. laughs at the suffering of the victim)
Lamentations 3:37-38:
"Who has commanded and it came to pass, unless the Lord has ordained it? Is it not from the mouth of the Most High that good and evil come?"

Satan does not know our thoughts, nor can he read minds. He has been alive longer than man has lived on the earth. He has used every trick to trip us up and knows what works. Think about it, he knows that if he places a flirtatious, gorgeous, naked woman in front of a married man he can probably get 3 out of 5 men to cheat on their wives. If he puts a poor man next to someones thick wallet and no one is around, chances are good he'll steal it. He's been watching us for a long time.

Only God can hear our thoughts, Satan can't. Satan knows our weaknesses by what we say out loud and by the ways we act.

Satan knows your weakness because most likely you are constantly saying what your weaknesses are. that is the only way for him to know. don't confess bad things about your self.

Satan lurks in your closet
and sometimes under your bed
so he can more easily listen
to the sinful thoughts in your head.
