If your medical expenses total more than 7.5% of your adjusted gross income, and the procedure was to address a mental or physical condition, then yes.

If the procedure is not medically necessary, then it generally is NON-deductible. Yes, a decent rule-of-thumb is the insurer's policy of coverage, but not always.

If a medical procedure is medically necessary, you may deduct it, but only the amount of your TOTAL medical expenses that exceed 7.5% of your adjusted gross income are deductible.

If you have cosmetic surgery that enhances your income, such as breast augmentation for an exotic dancer in a club, you have a business deduction, not a medical expense. FYI, breast augmentation is a 3-year depreciable item!

Surgery for cosmetic reasons is not deductible. If it's partly for cosmetic and partly for medical reasons, then it might be.

Yes it is tax-deductable, it is as tax-deductable as any other medical expense your insurance did not pay for. I am not certain of the percentage, but claim the entire amount.. It was an medical expense for you in 2006 therefore you may claim it, hell, you can write off bandages, so surgery is a definate YES.

I would imagine it depends. If the surgery was to "Treat a mental or physical illness" then yes it is deductible up to 7.5% of your income. Things such as vision correction and breast reduction surgery have been covered. Teeth whitening was not covered. Its all going to depend on what surgery you had.
