I am traveling to Madrid, Switzerland and Italy this summer. I would like to get custom shoes made without breaking the bank if possible. I will be in Florence and Rome in Italy.

I don't know about Spain but I know a lot about Italy and prices of custom shoes. Here just the very rich people can afford the cost of such a pair of shoe (cost goes from 500-600 to 1000-1500 €).-
In a previous answer I read about a shop at Capri; I don't know, it could be possible but however they're lady sandals and I don't think you are interested to buy them just because they're cheap price....!!In any case I don't understand why you're looking for 'out of series' shoes (unless you've orthopedic problems to yr feet).
Mainly in Florence (but even in Rome) you can find a real huge assortment of men shoes which maximum cost is around 200/300 € for a very good quality (but you can also spend less if you stay away from the tourist routes).

What do you consider breaking the bank? On the Island of Capri there is a shop called Canfora that makes incredible sandals... I had a pair ordered and shipped to me, and I LOVE them and will order more. They run about 150 - 200 Euro.

I don't think there is one. The only chance I see you having in creating your own shoe, is contacting the Nike design group and asking them. But even if you want to it will be very costly.
