Costantino is a Italian first name (=Constantine, as ancient Roman emperor).

Costantini is a family name derived from Constantino, putting a -i instead of -o: it's typical way of patronymic names (IE names derived from father first name):
eg Dante Alighieri, because his father was named Alighiero
