What is in his bedroom ? Is it full of stuffed animals and pets ? Have you changed laundry detergents recently ?Is there dust in hidden areas ?
Try and clean the bedroom as often as possible, use mattress protectors and pillow protectors.

Maybe he does not like his food.
Maybe some salty little crackers would taste good. Do you get "goldfish" where you are ... a little salty cheese cracker ... and I know it sounds like folk medicine, but would he like little sips of plain tea with sugar ... tea was originally used for medicinal purposes and has been known to settle down quite a lot of things ... maybe he would like a little chocolate, some warm cocoa or some eating chocolate ...
What does he say about it?
Does he have enough fun? Playtime? Maybe he hates this life... encourage him to do something he likes to do ...
Is it possible that he has a breathing difficulty that shows up only when he is lying down.... or some sort of congestion ... does the Dr. listen to him in that position? Do you encourage him to blow his nose?
I am out of ideas.

you said you took away milk, but what about wheat? That is another huge food allergy. However you will have to take away 90% of what he eats normally 'cause everything has wheat in it. What about acid-reflux? That was the first thing I thought of. I know the bills are high but I'd get a second opinion...maybe a dietitian.

Check for these possibilties:
1) Acid Reflux
2) Gal bladder
3) Gluten allergy-: it starts in children when they start eating foods with gluten (like wheat,barley and rye found in cereals and other foods for example) This allergy in children can cause vomiting.and sometimes diarrhea.
