The international access code varies from country to country. It is 00 in most countries. In the USA it's 011 - some countries have bizzarre numbers.
If you roam with a cell phone, then you may not be sure what the international access code is in the country you are in. Also you may wish to call from your address book.

So cell phone (well at least GSM ones) recognise + at the universal international access code and will work in all countries

cause they might have relatives that live overseas and have it automatically recognise the ph.
